Risk surveys evaluate risk in change orders associated with change categories. The survey lists a series of single or multiple choice questions. Each answer has its own weightage value. When creating a change order, the user selects the appropriate answers and submits the survey. The risk level is based on weightages of answers selected by the user.

Specifies the risk survey description.
Indicates if the risk survey is active or inactive.
